Please use this form to register for the "Food for Thought" workshop. The workshop is for teachers from grades 2 - 8 and will be held Tuesday, February 17, 2009 at the NM Educator's Federal Credit Union Training Center from 8:30 - 3:30 pm.

Note:

We have received 15 registrants who require a substitute, the limit of our funding. If you wish to place your name on a waiting list, please submit your request and if cancellations occur, you will be contacted in the order of your registration as a replacement. We can take a limited number of registrants who do not require a substitute teacher to attend the workshop. Please submit your request for a place in this exciting workshop if you are interested!

Note: The workshop is limited to 15 participants if a substitute is required. We can accept a total of 20 participants if 5 individuals do not require a substitute. Reservations will be accepted on a first come/first serve basis, with additional requests placed upon a waiting list. If you register and find yourself unable to attend, please notify Donna Moore (moore_d@aps.edu) so she can add someone from the waiting list. Successful applicants will be notified by email and/or phone.
